Emergency and Daily Medical Care Forms
Below are links to the lists of the various forms needed to enroll your child for the upcoming School year. Please read through this list carefully and complete all forms applicable to the grade your child is entering. All forms must be returned by the dates indicated. Thank you for your cooperation in this important process.
Note that some of these are medical forms which require an up-to-date physical.
NOTE: Some forms require a physician's signature (stated where necessary). It is essential that School families make their doctors' appointments as soon as possible in order to meet completion deadlines for each form.
Some forms (see below) must be received by the School no later than June 1, 2008. Your child's report card will not be sent until all of these forms have been received by the School.
Student Emergency Contact and Directory information must be submitted online by June 1, 2008. Online submissions began on May 1, 2008.
Medical forms for all students are due by July 15, 2008. Students will not be allowed to attend pre-season practices, participate in orientation and/or begin School if their medical forms have not been submitted. All immunizations must be up-to-date as required by law.
